Don't believe your LDL cholesterol!

Harry's case is typical. For years, his doctor told him his LDL cholesterol of 123 mg was okay. But a heart scan score of 490 (90th percentile at age 52) made him question just where his coronary plaque came from.

Lipoprotein analysis told a very different story: His LDL particle number was 2400 nmol, meaning his trueLDL was more like 240 mg, nearly double the value of LDL obtained through his doctor. Harry had other sources of risk, too, but the LDL particle number was a clear stand-out.

Why does this happen? How can LDL cholesterol be so terribly inaccurate?

LDL cholesterols obtained in virtually all labs are not measured, they're calculated. The calculation was developed in the 1960s by Dr. Friedewald at the National Institutes of Health and therefore goes by his name (the Friedewald calculation). Dr. Friedewald derived this simple calculation to permit doctors across the U.S. to obtain LDL cholesterols, which were technically difficult to measure in those days by using measured HDL, total cholesterol and triglycerides.

Doctors were told that the only time that the Friedewald calculated LDL was inaccurate was when triglycerides exceeded 400 mg. So most family practitioners and internists still believe that calculated LDL's are, for the most part, quite accurate.

Nothing could be further from the truth. When LDL's are actually meaured, you find that LDL is rarely accurate. In fact, in our experience, inaccuracy of 30-50% is the rule, sometimes 100%. The one telltale hint that calculated LDL is wrong is when HDL is <50 mg--that's nearly everybody.

So what's your LDL? You won't really know unless it's measured. Our preferred method is NMR (LipoScience) LDL particle number, probably the most accurate of all. Second best: apoprotein B, direct measured LDL, and non-HDL. (We'll cover this issue much more extensively in an upcoming report on the www.cureality.com website in an extensive Special Report.)

Baby your pancreas

There it is, sitting quietly tucked under your diaphragm, nestled beneath layers of stomach and intestines, doing its job of monitoring blood sugar, producing insulin, and secreting the digestive enzymes that allow you to convert a fried egg, tomato, or dill pickle into the components that compose you.

But, if you've lived the life of most Americans, your pancreas has had a hard life. Starting as a child, it was forced into the equivalent of hard labor by your eating carbohydrate-rich foods like Lucky Charms, Cocoa Puffs, Hoho's, Ding Dongs, Scooter Pies, and macaroni and cheese. Into adolescent years and college, it was whipped into subservient labor with pizza, beer, pretzels, and ramen noodles. As an adult, the USDA, Surgeon General's office and other assorted purveyors of nutritional advice urged us to cut our fat, cholesterol, and eat more "healthy whole grains"; you complied, exposing your overworked pancreas to keep up its relentless work pace, spewing out insulin to accommodate the endless flow of carbohydrate-rich foods.

So here we are, middle aged or so, with pancreases that are beaten, worn, hobbling around with a walker, heaving and gasping due to having lost 50% or more of its insulin-producing beta cells. If continued to be forced to work overtime, it will fail, breathing its last breath as you and your doctor come to its rescue with metformin, Actos, Januvia, shots of Byetta, and eventually insulin, all aimed at corralling the blood sugar that your failed pancreas was meant to contain.

What if you don't want to rescue your flagging pancreas with drugs? What if you want to salvage your poor, wrinkled, exhausted pancreas, eaking out whatever is left out of the few beta cells you have left?

Well, then, baby your pancreas. If this were a car with 90,000 miles on it, but you want it to last 100,000, then change the oil frequently, keep it tuned, and otherwise baby your car, not subjecting it to extremes and neglect to accelerate its demise. Same with your pancreas: Allow it to rest, not subjecting it to the extremes of insulin production required by carbohydrate consumption. Don't expose it to foods like wheat flour, cornstarch, oats, rice starch, potatoes, and sucrose that demand overtime and hard labor out of your poor pancreas. Go after the foods that allow your pancreas to sleep through a meal like eggs, spinach, cucumbers, olive oil, and walnuts. Give your pancreas a nice back massage and steer clear of "healthy whole grains," the nutritional equivalent of a 26-mile marathon. Pay your pancreas a compliment or two and allow it to have occasional vacations with a brief fast.

    Pretty good advice, doctor.

    The Centers for Disease Control has predicted that one of every three Americans born in 2000 will eventually develop diabetes (mostly type 2).  Your dietary advice, along with regular exercise and avoidance of obesity, would prevent many cases.

    Great article! Yes, our pancreas is neglected badly. It is working hard, day in, day out and we don't even notice this hard labor.
    One can measure if the pancreas is still working ok by measuring "C-Peptid". A sensitive marker of insulin production. Mine is 1.0 (a little low), so I want to support my pancreas.
    Does anybody know how to increase or re-generate beta-cells?
    There is a herb (among others), called GYMNEMA SYLVESTRE that has supposedly regenerative capabilites. Has anybody tried this?

    Doctor Davis:
    No rice or potatoes?  Are you advocating a zero carb (except carbs found in veggies and fruit)?  Many of us are allergic to nuts so foods like walnuts are out, but peanuts are ok since they are legumes, but many seem to say stay away from them.  Be helpful to get more detail on your daily diet suggestions.

    Doesn't the pancreas work to produce insulin and glucogen even on low-carb meals?  I thought the problem with high-carb diets was the elevated level of insulin in the blood and the various metabolic syndrome problems that causes, not necessarily that it overworks the pancreas.

    Agreed!  Please correct me if I'm wrong, but isn't there the a danger that as a person gains weight and the body is experiencing continuing high insulin levels, some tissues become insulin resistant, so the pancreas is forced to produce  even more insulin to keep the glucose levels in the blood under control.  That, I understand, becomes a  "slippery slope" and difficult to recover from.  I don't mean to be alarmist - but it is downright scary.

    FWIW: I was spiking to 185-210 at one hour post prandial on Dr Fuhrman's greens/beans etc diet.  I added Gymnema from Pure Encapsulations Gymnema sylvestre extract (leaf) (75%)250 mg, one capsule, 3X/day and after about 5 months my highest spikes have been in the low 120's after BIG meals, same diet, no grains but plenty of fruit and some root veg and lots of beans.  Even a good sized serving of oatmeal only get my sugar reading to the mid 120's. Seems to me, likely some beta cell rejuvenation has gone on.  How much the gymnema and how much the diet is anybody's guess.

    There's no evidence that using your pancreas is what leads to reductions in beta cell mass and function.   Have you looked at any of the peer review research demonstrating that drugs like Byetta can actually increase beta cell mass and increase insulin sensitivity?

    2,000 - 3,000 Beta cell work to coordinate insulin put out every +/- 4 minutes; post-prandial there is a 1st response insulin spike, then smaller steady secretion, and in time a lesser 2nd insulin spike.  Some Type 2 diabetics don't  put out the 1st insulin spike, their blood sugar stays high and then eventually they do put out that late 2nd insulin spike.

    "Amylin"  or IAPP (islet amyloid poly-peptide) is co-secreted with all insulin;  if  "amylin" goes on oligomerize into a  tangle of fibrils around the individual Beta cells this messes with their signal synchronization.  As the "amylin" kinks up with other amylin the Beta cells are pushed apart and those Beta cells change size; without Beta cell group co-ordination the individual cells' mitochondrial house keeping repairs risks compounding recycling errors and that cell goes into programmed cell death (apoptosis).

    Zinc is what binds to an individual "amylin" fibril  in a central position and keeps that fibril from oligomerizing with other fibrils; too little zinc in pancreas lets tangles occur. Yet too much zinc in pancreas also lets tangling get going; this is due to when a 2nd zinc clings elsewhere on  the same fibril the first zinc already is bound to.  See  recent "Journal Molecular Biology", vol 410, 2, 294-306

    Damned interesting study if it bears out. It seems that a couple months of hard starvation lowers fat levels in the pancreas and can actually reverse diabetes, at least in people who are not too far along in the disease process.

    It does not take anything like the starvation in the above linked study to reverse insulin resistance and effectively eliminate T2 diabetes.  There are a number of doctors who have been having that result for many years w/ a lowish fat, plant based diet, eliminating REFINED carbs, animal fat, saturated fat, and manufactured junk food.  See Fuhrman, McDougall, Barnard, Esselstyn, Shitani, etc. They all have books out and a track record easy to find.

    No need to starve to "cure" type 2 diabetes.  Just cut the carbohydrates, particularly the starchy kinds from grains, legumes, and potatoes.  No need to eliminate animal fat/sat fat, either. Natural fats have no effect on blood glucose and do not stimulate insulin production.  If you eliminate all the things Fuhrman et al say to eliminate, there isn't much left to eat and you will end up on a starvation diet anyway!

    Might some of Doc's diabetics be getting their relief from eliminating wheat be due to more zinc absorption (and thus less "amylin" tangles) ? In theory  phytates  have the potential to excessively bind up zinc; although I hesitate to say one scenario fits all situations.

    Zinc deficiency in the pancreas is hard to assess, since we use zinc all over ; specific tissue levels are a guess,  because we are physiologically designed to keep a baseline amount of zinc in our blood. In other words,  zinc can actually be retrieved from body tissue and put back into circulation;  maybe  some individuals have a disposition to leach zinc out of their pancreas and then their "amylin"  tangling worsens.  Constant adult doses of over 100 mg per day really merits caution and, aside from other side effects, may tag "amylin" fibrils with 2nd zinc that provokes  "amylin" tangles; I am not suggesting any specific dosage for anyone here.  

    Meanwhile,  other individuals possibly avoid Beta cell death due to genetic propensity to maintain enough zinc in their pancreas;   and,  also some may have gut bacteria strains that slowly pass the  intestine zinc the bacteria cleaved from phytates' hold.  Not every overweight person, nor  grain/legume/nut phytate  consumer goes on to develop diabetes; there may just be some age related Beta cell decline  that is normal and not pathological.

    The whole notion of beta cell regeneration, while tantalizing, is uncharted territory. I propose carb-limitation not so much for its beta cell regenerating potential, but for its ability to simply not challenge a compromised pancreas.

    Might-o-chondrial's proposal that grain phytates block absorption of zinc is interesting. However, I don't know how much of a genuine role it plays in the entire picture. I can say that the end clinical effects include HbA1c's of 5.0 or less, fasting glucoses of 90 mg/dl or less.

    Lack of imagination/creativity is a terrible thing.  The only thing Fuhrman et al say to eliminate is manufactured, highly processed, refined, chemicalized, "food like substances" (that are not really food at all).  I eat basic Fuhrman diet and can easily adjust my calorie intake up or down by 1000 calories or more just by substituting higher or lower calorie density real foods.  It isn't difficult, and even at lower calorie intake, no sense of deprivation at all after the first bit of breaking addictions and retuning taste buds.

    60 - 80% of new Type 1 diabetics show anti-bodies for Zinc Transporter 8; there are 10 different zinc transporters (ZnT) in mammals.  ZnT works with the movement of zinc from a cell's cytoplasm to sites of action inside that cell and also moves zinc outside that cell (bringing zinc into cell is not directly mediated by ZnT).

    A genetic polymorphism of ZnT 8 (SLC 30A8  rs13266634) causes less pro-insulin made into insulin (crystalization of insulin for stockpiling  involves zinc)  and when high blood sugar malfunctions;  if less insulin
    stash in insulin storage granules then it's secretion on instant demand (ex: 1st post-prandial insulin pulse)  is impaired.  Normally lots of ZnT 8 is active in pancreatic  Beta cells and actually prevents Beta cells from dying off due to zinc deficiency;  as well  as  lots of ZnT 8 activity expression inducing more  insulin put out in response to blood glucose loads.

    High blood glucose down-regulates ZnT8 and , on the contrary, up-regulates ZnT 3;  Doc is right to be concerned with excessive post-prandial blood sugar because ZnT3 and ZnT8  have different dynamics.  ZnT3 is linked to amyloid tangling in the brain (hippocampus has lots of ZnT3) and recent research find it is active in the pancreatic islets; so I wonder if it's transporter genetics (gene SLC 30A3) is how  zinc becomes a problem outside the cell,  and integral to how "amylin" fibrils then can kink into tangled oligomers that push Beta cells too far apart.

    Glad to see T. H.  back .... for Melatonin synthesis we use zinc and taking zinc raises one's melatonin output (2003  journal Acta Physiologica Hungarica, 90(4),335-339).  Pancreatic lipase enzymes that help cleave fat we've eaten also  uses zinc.

    I have no zinc guide lines for anyone, and definitely  am not claiming zinc is the only relevant parameter in pancreatic dynamics.  For home evaluation  of zinc status please see Dr. Dereck Bryce-Smith's  "ZTT"  technique; it is a 10 second mouth swish of 2 teaspoons of zinc sulfate mono-hydrate and how one's response ranges in four gradients  going from tasteless to metallic.

    I am just about convinced that I have experienced some amount of beta cell duplication as a result of many months of low carb dieting, as noted in an earlier post by Might-o in March regarding the formation of new, larger beta cells in the absence of hyper-glycemia.  After trying many diets to address my type 2 diabetes, I settled on a low carb diet of 65% fat 25% protien and 10% carb after extensive research on the effects of the different micronutrients on blood sugar.  For almost a year, my A1C has been 5.4, and I lost 40 lbs in that time period.

    However, after feeling that I had lost some strength in weightlifting, I did some additional research on maintaining strength while on a low-carb diet, and it was suggested that on weekends, to eat anywhere from 100-500 grams of carbs to restore my glycogen levels.  To make a long story short, I ate half of a large pizza, and tested my blood glucose before and one hour after.  Before, it was 82mg/dl.  After? 67 mg/dl.  I tested this again the next day with a few tacos, which would have normally shot my blood glucose numbers in the 100's and it produced the same effect.  

    One concern that I have is that my pancreas is producing excessive insulin, which is a problem for us diabetics who have been abusing our pancreas for so many years.   However, I believe that I have recovered some insulin sensitivity by giving my pancreas a rest and keeping my insulin levels low.  Does that mean that I'm cured and I should start gorgeing on pizza, cakes, sodas, french fries, mashed potatoes, pancakes and fried chicken like I used to? (I know, I brought diabetes on to myself)  The answer is no.  But it is a good idea to preserve our existing working beta cells by giving it a rest.

    Some nutritionists & dietitians recommend no more than 10 or 12 teaspoons of  *added*  sugar a day, as in addition to your regular daily diet, others say 10 to 12 teaspoons total sugars per day, with no additional.

    Since only 1 percent of your pancrease is used to produce insulin to process blood sugars, and since the average human body contains 5 liters of blood which is equal to around 1000 teaspoons, and a homeostatic fasted blood sugar level of 60 to 90 mg. per deciliter equates to a little less than 1 teaspoon, the theory is the less sugar consumed is the better option.  A handy formula to memorize is the 1-5-20 rule, which means 1 teaspoon of sugar equals 5 grams which equals 20 calories, then just do a little multiplication or division to convert the grams & calories of carbs on food labels into a teaspoon equivalent that's easier to wrap your brain around, and as a good visual aid to understand how much glucose you're dumping on your pancreas all at once.

    I seriously doubt Dr. Davis advocates *NO* carbs, very few people that understand how to exploit the macronutrients do, especially doctors.  Most people have poor long term results & can't sustain a food denial strategy, so rather than think in terms of carb denial it's better to just have limited amounts of the carbs you like while keeping in mind the total glucose content, glycemic index and glycemic load of those foods and the stress placed on your pancreas by overindulging.

    Just remember that carbs have 4 calories per gram, so to get the total calories by carbs per day just multiply the grams by 4.

    I was wondering if you could plan a future post around Microvascular Disease, particularly in women.  I understand the symptoms are different, and was hoping that you may be able to shed light on which lifestyle factors are most effective.  I'm seeing more and more thin, active women (many younger than 65) with "perfect" lipids in cardiac rehab.

    I have been taking metformin for polycystic ovarian syndrome. I have been low-carb/primal for a year now. Blood work (sugar and trig/hdl) look great. Any idea what I am doing to my pancreas by continuing the metformin? I have already decreased the dose since my blood sugar was getting too low. How do I know if I don't need to take it anymore? I am not taking metformin to control blood sugar.

    Another way to baby your pancreas is to avoid, where possible, dysglycemic drugs--two examples include: quinolines (antimalarials) and fluoroquinolones (FQs, e.g. ciprofloxacin, levofloxacin, moxifloxacin).  Bactericidal antibiotics produce more oxidative stress than do bacteriostatic antibiotics.  FQs produce a lot of oxidative stress, probably more than others and they are handed out like candy.  FQs also deplete and mutate mammalian mitochondria.  

    Interestingly, B-cells in the  pancreas have very low levels of antioxidant enzymes (glutathione peroxidase and catalase both mop up reactive oxygen species)  comprising only 1% of the levels in the liver.   This favors H2O2 accumulation which can mess up H2O2 signal transduction.

    I wonder why our pancreas B-cells  do not have more antioxidant protection than do cells in the liver?  Maybe the difference is the amount of zinc and iron between the pancreas and liver?  Zinc is a potent antioxidant by itself and a cofactor for the third major antioxidant enzyme of our bodes--super oxidase dismutase.  Iron is highly risky for oxidative stress particularly in combination with a hydroxyl radical--this is an explosive combination.

    How much the rapid increase in diabetes is associated with  the overuse of prescription medications that are tough on the pancreas?

    I have references for all of the above if anyone is interested.

    You may be interested that it is MnSOD (manganese superoxide dismutase) which makes risky super-oxide molecules into molecules of H2O2 (hydrogen peroxide); the net effect of having more MnSOD activity is lower levels of super-oxide but more H2O2.

    Hope you have previously tested those same foods at 30 minute intervals because blood glucose peak timing can vary greatly depending on specific food/meal and individual physiology. Pizza is one of the worst for long delayed but very high peak

Faye was clearly frustrated.

At age 52, she was having chest pains every day. A CT heart scan showed a score of zero. A CT coronary angiogram showed no plaque whatsoever.

"Everything went downhill when my menopause started. I gained weight, I started to have chest pains, my blood pressure went up, my cholesterol shot up."

She saw three physicians, none of whom shed much light on the situation. They ran through the predictable sequence of (horse, not human) estrogens, anti-depressants, suggestions for psychological counseling.

But we checked Faye for lipoprotein(a), which she proved to have at a high level of 182 nmol/l. This explained a lot.

A curious and predictable set of phenomenon occur to females with Lp(a) proceeding through the menopause. As estrogen recedes:

--Lp(a) levels rise dramatically.

--Blood pressure goes up, sometimes creating severe hypertension by mid- to late-50s.

--Chest pain can develop, presumably due to "endothelial dysfunction" or "microvascular angina", both representing abnormal coronary artery constriction facilitated by worsening expression of Lp(a).

All too often, these phenomena get dismissed as simply part of the menopausal package, when they are, in fact, important facets of this very important genetic pattern that confers high risk for heart disease.

If any of this rings familiar for you or a loved one, think Lp(a). Though Faye hadn't yet developed any measurable coronary plaque by her CT heart scan score, it was likely on its way, given the surge in Lp(a) expression as menopause unfolded--unless its recognized and appropriate preventive action taken.

Who is your doctor?


Primary care physicians are the initial entry point for healthcare for the majority of Americans.

Develop pneumonia; go to your family or internal medicine physician (internist) to be prescribed an antibiotic. Need your blood pressure or cholesterol checked? Develop a sore knee or swelling in your leg? Once again, go to your primary care physician.

Primary care physicians are a patient’s guide to a bewildering array of technology and specialists. If you require a specific diagnostic test or consultation with a specialist, your primary care physician will help you navigate through the maze, choosing the path that is best for you. He or she will order a chest x-ray for a cough and fever, provide vaccines to prevent flu or pneumococcal pneumonia, perform an annual physical. If you require hospitalization, your primary care physician will admit you. He or she will order diagnostic tests like MRI’s, ultrasounds, x-rays, and blood testing, usually performed in the hospital or a hospital-owned facility. If you require the services of a gastroenterologist, orthopedist, general surgeon, or neurologist, your primary care physician will refer you to the appropriate specialist.

That’s how it’s supposed to work, at least in principle. In fact, during the first eight decades of the 20th century, it did work that work way for the most part. Your primary care physician acted not just as a provider of healthcare, but as your advocate, someone who knew you and worked to protect your welfare. Your family doctor often knew your parents, maybe even delivered you at birth, and cared for your children. His children often went to the same schools as your children. He and his family lived in the same town and sometimes went to the same church.

That hardly happens any more. It’s more likely you got the name of your primary care physician from a doctor referral service provided by a hospital. Or you picked a name off a list provided by your health insurer. It’s also common to see one doctor, only to see another a year later. Two, three, or more different primary care physicians over a five-year period are common. Doctors come and go, since physician turnover in clinics and practices has been on the increase for years. Insurance companies frequently force policyholders to change doctors, requiring you to choose from a list.

The end result of this shuffling of primary care is increasing impersonality of the relationship. You probably don’t know your primary care physician outside of the 10-minute interaction you had six months ago. She probably never met your mother and will likely not care for your children. Two years from now, she will likely not be your doctor any more, replaced by someone else who obtains the details of your health from a chart. Your chart is more likely to be electronic, with the details of your health history listed in a checklist. There’s little room to detail the idiosyncrasies and quirks of your unique personality or health profile. Throw into this impersonal equation the fact that many doctors have become scared of patients because of potential for lawsuits, often over the most trivial of issues, or because of an error of oversight or misdiagnosis.

This flawed and impersonal system, though emotionally unsatisfying, can still work if each doctor who assumes a patient’s care maintains the ethic of putting health and welfare above all.

But what if your primary care physician is not just an advocate for your welfare, but is a representative of the hospital? What if there are hidden, unspoken financial incentives paid to your doctor to direct you to the hospital for diagnostic testing, hospitalization, and referral to specialists? If a headache becomes a $4800 MRI, or chest pain becomes a $4200 nuclear stress test, then a $14,000 heart catheterization, your primary care physician becomes the purveyor of far greater financial opportunity for the hospital. The entire interaction, founded on the proposition that your doctor actually cares about you, collapses in a heap of financially motivated testing and procedures. It appears to work, and you and your family can still obtain access to healthcare. The problem is that you’re likely to get too much of it.

This message has not been lost on the shrewd administrators at hospitals. Take a look at the ranks of primary care physicians who refer patients to some of your local hospitals. It is typical that a hospital system maintains several hundred primary care physicians on their payroll, all of whom are expected to refer patients to the hospital, cardiologists, and other proceduralists. Why so many?

Most primary care physicians today have signed contracts with a hospital. In other words, they are employees of the hospital. This practice is not unusual: the American Medical Association reported that 4 of 5 primary care physicians are now bound by such employment arrangements across the U.S. In effect, 80% of primary care physicians are legally bound by contract to direct patients to cardiologists who work at hospitals.

On top of contractual obligations, there are financial incentives for the volume of procedures that are generated as a result of referrals. The more procedures generated from an internist’s or family practitioner’s practice, the greater the end-of-year productivity bonus will be, not uncommonly totaling tens of thousands of dollars. Dr. Ted Phillips (not his real name, since he declined to allow me to use it) received a bonus check of $9,437 this year for his “productivity,” defined murkily as the return on specialist referrals. While the bonus may have helped him pay for his son’s college tuition, it clearly was a situation that made him acutely uncomfortable when asked.

Several primary care physicians are also quietly dismissed every year from the ranks of employed physicians for not maintaining a minimum flow of patients into the system.

Another hazardous point of entry: Many patients enter the hospital through the emergency room (ER). A patient in the emergency room is at his or her most vulnerable, seeking help for an urgent complaint and usually willing to accept whatever the ER physician advises. Hospitals know this. That’s why many systems insist that the ER physicians be employees of the hospital, with their practice habits subject to control. A patient goes to the ER with chest pain or breathlessness. The worst thing that can happen from a financial standpoint is for the patient to be evaluated and discharged. For this reason, a growing number of hospitals employ ER physicians, then proceed to legislate practice patterns. Consulting a cardiologist is strongly encouraged, since they generally provide access to the downstream revenue-producing procedures offered in the hospital. That way, what might have been a four hour, $2500 ER visit is converted into a $10,000 to $40,000 hospital stay, even when nothing was wrong in the first place. There are millions of people nationwide who have the hospital bills to prove it after being discharged with a diagnosis of indigestion.

Caveat emptor: Buyer beware.

    Wow!  This is an incredible conflict of interest!!  No wonder I have felt so much pressure to do this and that despite never being sick and having no symptoms.  

    How did medicine get to this point?  The better question is 'How can doctors live with themselves?'  

    We all know that everyone has to earn their living but but this milking of the system and of patients is truly horrible! It is especially insidious when you are dealing with someone you need to trust implicitly and who tells you that you need to do these things because he/she is worried about your health!

    As an advocate of free market solutions, I will be looking for a physician in my area with the same philosophy.  But I don't really expect to find one, as I am in an area dominated both by a State University Health System, and a Private University Health System. My own PCP is in the private University system.  We are asked to sign arbitration agreements with the University in case of  some untoward event (I refused, but I believe many patients believe they must either sign or go untreated, or else do not realize what they have signed.) Now the system has implemented a program that is ostensibly designed to help patients develop preventive health practices.  You are provided a liason person ( a nurse I assume) to check in with once/month by phone,and in that conversation you are asked about your goals and what you're doing to reach them, and given the usual status quo, conventional advice about concerns.  (This person will never even set eyes on you, after all.)   Still, it  doesn't sound bad, even sounds as if they are making genuine if nonsubstantive efforts toward helping people be proactive.  But I would not be surprised if the whole scheme were somehow rigged to the advantage of the University insurance plan, such as being enabling them to see if you are receiving more treatment (such as seeing an acupuncturist or chiropractor) than "they" think is appropriate or taking only the drugs "they" approve, and I also suspect that one day it will no longer be voluntary, and that you will be coerced toward only the practices they approve of.  Why are they offering payments for participation, is my question, and what do they do with the information you give them?  I will be opting out of that too.

    As far as free market solutions and the concierge model go, many will argue that it is too expensive and will only provide care for an elite.  The Arizona physician will charge a flat fee of $1500/year, and that will entitle his patients to direct access to him by phone or email, and at least 30 minutes of his time per visit.  His practice will have to be smaller than in the past in order for him to give more to each patient, but he will be able to actually practice medicine rather than be a cog in a corporate machine. Surely there are free market models that can be devised for the average person as well.  To object that the cost of modern health care is prohibitive may be simplistic.  Why is it prohibitive?   Can it be because the market does not function freely under the triple chokehold of insurance, drug company and government collectivism?    If patients and physicians were able to devise alternatives to the present environment of health care and PARTICULARLY if true, effective preventive practices (and many are already available at nominal cost but not promoted) became the norm, costs could plummet.  Hail to the pioneers such as Dr. Davis and the TYP members, and to the Arizona Concierge physician, who are striking out for new territory.  We will all benefit tremendously.

    Your fellow cardiologist Rich Fogoros also addressed this breakdown of the role of doctor as advocate, but he argues just the opposite: that PCPs today are essentially employees of the health insurance companies, tasked with controlling costs by minimizing tests, procedures, specialist referrals, etc., like claims adjusters.  His version certainly seems to be more true where I live.  I can't speak to the extent to which my PCP is under pressure to stay within a quota (however informal) for monthly rheumatologist referrals, CT scans, etc., or the degree such pressure taints his patient interactions, but I do know that he works in a clinic owned by the same (insurance) company to whom my employer and I pay my premiums, and that the same company also owns the local hospitals.  Clearly the profit-maximizing thing in such case is to maximize (healthy) policyholders and minimize claims (treatment).  There's no profit in gratuitous caths and bypasses when the hospital is the insurance company.  Fogoros claims that this sort of vertical integration became the rule when the HMOs bought up all the community hospitals and clinics in the late '90s.  A friend of mine (under my same plan) suffered a nasty GI tract problem and had to beg and fight her PCP, over 2-3 months of almost constant vomiting and other fun, for a referral to a gastroenterologist, who did finally diagnose and cure her.  The PCP was clearly in over his head, yet kept futilely giving her the same garden-variety pills (antiemetics, PPIs, etc.) and at one point told her, "Look, I can't just send everybody to the GI to have him say, 'Take the Purple Pill!'"  So Fogoros' "covert rationing" seems to go on in my town.  Can you reconcile that with what you're saying?

    All my adult life I've been told by doctors I have "borderline hypertension" (about 140/90). None, however, ever even suggested that I take an anti-hypertensive. Now I have atherosclerosis and can but wonder what might have been. Let mine be a cautionary tale.

    Not all insurance plans require a person to go through the PCP for referral to a specialist. I have seen patients who are seeing multiple specialists where it seems no one is in charge of the overall picture. These people may be given duplicate tests and incompatable medications. Doctors don't always communicate with each other. Shouldn't there be one chart that follows the patient from doctor to doctor?

    When I was seeing a university PCP I found it difficult to get tests such as vitamin D, B12 and A1C.

    Practicing primary care in the US in the current environment is incredibly challenging, but still very rewarding. As an employed family physician in Massachusetts, I am pulled in many competing directions: the hospital encourages referrals and admissions into the system the patient's insurance company discourages some referrals and limits access to certain medications and expensive tests.  There is more paperwork and bureacracy than ever.  Patients are more informed now, and often come in with information, some of which literally may be based on one person's opinion from a website or blog.  There are often many issues to try to sort through in one visit, a visit which is often limited by time pressure that exist as doctors try to see more and more patients.

    Despite this, I still feel it is a privilege to be chosen as someone's family doctor.  I tell my patients that I will try to treat them like my family and give them the same advice I would give my mother or my daughter.  I see my role as their guide, and I try to lay out the different options, and then tell them which one I would personally choose, but that it is ultimately their body, and they get to make the decisions.

    What it comes down to for me is that medicine is still about the interaction between the doctor and the patient, and if a patient is unhappy with that interaction, then they have the right to try to find a new doctor where they will feel cared for.

    I had to also respond to Jenny's post about concierge medicine.  It is definitely an attractive option for some patients and physicians.  I once read it explained like this: "Imagine you're a plumber, and someone says I will pay you more money to do fewer plumbing jobs, so you can spend more time on each job, and really focus on doing the highest quality work you can, really get back to what it was that drew you to being a plumber."  Who wouldn't want that?  My problem is that it is not extrapolable.  If a primary care concierge physician only has 300 patients, there is going to be a huge shortage of primary care doctors in this country.  My training in both medical school and residency was oriented around taking care of all the patients that needed help, not just the few hundred that could afford an annual fee of several thousand dollars a year.

    There is a group of doctors who have come together in Seattle to practice medicine in a very intriguing model.  It is called Qliance (www.qliance.com).  It has some similarities to a concierge practice, but is much more affordable.  They don't take insurance, there are direct monthly costs that are charged to the patient which covers unlimited visits.  Additional tests are paid for by the patient.  This model financially aligns the patient and physician, and the usually-hidden costs of medicine are out in the open.  One physician who works there described it as very refreshing to feel that you are really working for the patient.  It won't be the answer for everyone, but it may be the answer for many.

    Anne posted that some patients are seeing multiple specialists without having a primary care doctor.  

    That's my predicament, and I wish it weren't so.  As an 57 year old woman with multiple annoying health problems including diabetes, hypertension, and some difficult-to-treat chronic pain (chronic daily migraine, neck problems), I can't find a PCP who will stick with me: They are apparently so unnerved by the list of maladies that I must deal with that they decide I must be crazy, not ill, and stop working on my behalf.  

    I just hate trying to be my own PCP!  I'm terrible at it!  The idea of having a PCP who would help me manage all this stuff is a dream!  And I haven't had a physical in over a decade because I can't keep a PCP.  And although my specialists kindly try to fill that gap in my treatment, I know that not having a PCP is dangerous.

    However even with my excellent insurance and ability to pay and living in an area with an unusually high number of doctors/capita, apparently I fall into a category of patient that PCP's just can't deal with.

Matt came to me because his doctor couldn't reduce his LDL cholesterol.

His doctor had prescribed Zocor (simvastatin), Lipitor, Crestor, even pravastatin, all of which resulted in incapacitating muscle aches and weakness within a week of starting. No surprise, Matt had a jaundiced view of statin drugs.

We started out by characterizing his lipoprotein patterns:

--LDL 155 mg/dl

--72% of LDL was small LDL, a moderately severe pattern. (This means that small LDL comprised 112 mg/dl of the total 155 mg/dl LDL; large LDL comprised 43 mg/dl--small LDL was the problem.)

--HDL 42 mg/dl --Triglycerides 133 mg/dl

--No lipoprotein(a)

Beyond lipoproteins, Matt proved severely deficient in vitamin D with a starting level of 18 ng/ml.

Matt's doctor had advised that he avoid salt, as his blood pressure had been borderline high. His thyroid assessment disclosed a TSH of 3.89 mIU/ml with thyroid hormones free T3 and free T4 in the lower half of the normal range.

I therefore asked Matt to:

--Eliminate wheat, cornstarch, and sugars to reduce small LDL
--Add iodine
--Supplement 6000 units of an oil-based vitamin D preparation
--Take fish oil to provide at least 1800 mg EPA + DHA per day
--Take Armour Thyroid 1 grain per day


Several months later on this program, Matt had a repeat basic lipid panel:

--LDL 82 mg/dl--a 47% reduction

--HDL 52 mg/dl a 24% increase

--Triglycerides 60 mg/dl--a 55% decrease

In addition, vitamin D was 66 ng/ml, TSH was <1.0 mIU/ml with free T3 and free T4 in the upper half of the "reference range." Matt also felt great.

While the numbers could be slightly better, Matt had made tremendous progress towards achieving perfect values.

There you have it: Marked correction of cholesterol values, no statin drugs involved.

    My total cholesterol was 236 two months ago, and my HDL was 155. My numbers have been like this for fifteen years or so (I'm 45), but when they lowered the threshold for prescribing a statin from 240 to 200, my doctor wanted me on a statin. I got a CAC scan and got a score of 42, so I knew I had to do something. I decided to try diet changes first, eating a lower fat diet but still eating moderate amounts of meat. I also took fish oil, Vitamin D, and a multivitamin/mineral supplement. Today, I got the results from my latest blood test, and my total cholesterol is 162, LDL 95, HDL 47, triglycerides 90. The doctor didn't mention taking a statin this time. I plan to get another CAC scan after a year.

    Thanks for the post Dr.

    In my case when I started on fish oil capsules for Omega 3, my Uric Acid increased from 5.5 to 8.0.

    In this case, can I go for Cod liver oil? Does fish oil and Uric Acid have any connections based on your experience?

    How long, do you suppose, will we have to wait for a study comparing outcomes for statin use vs. a regime that includes fish oil, vitamin D, niacin, and a wheat-free, sugar-free diet? If we ever see such a study, will it make any difference?

    It was just announced that in Canada we spend $2 billion a year on statins, and I can't tell you the number of times I've printed off "Evidence for Caution: Women and Statin Use" for friends whose physicians want them to take statins.

    Any increase from the improved thyroid function is very likely offset by the reduction in total blood volume that follows a normalization of insulin levels.  When your insulin levels drop, your kidneys stop retaining sodium and your blood pressure will usually fall.

    I have had a remarkable reversal of my ill health, due to the diet of evolution, as you so eloquently proselytize for.  I just had to replace my Cardiologist, because he chooses to ignore your principles and practices.  My Nephrologist, Dr. Kenneth Tourgeman, who reads you every day, writes as you do, a very riveting and informative blog "nephropal.blogspot.com" stopped many of my medications including Staten's, and of course raised the hackles of my old Cardiologist.  He has since referred me to a Cardiologist with his and your same views.  You and he, along with a few other brave hearts are fighting the good fight. Health care through low carb and a healthy high saturated fat diet, supplemented with high dose Vitamin D3, high dose fish oil, and super Vitamin K2.  Keep up the great work. We out here in the medical wilderness, give thanks for you revolutionary doctors who are curing disease instead of just treating it.

    Kurt - your doctor is a mindless statinator. Was your HDL originally 155 - that is astronomical (or was that LDL).  Explore Dr. Davis's trackyourplaque website and then join.  You should get HDL up above 60, and trigs down to 60 or less.  Cutting wheat and fructose out or largely so will kick the Hell out of the trigs, and there are many other things to consider for reducing plaque besides statins.  Your heart scan scores are far more important as a measure of heart health than your LDL number, and your second heart scan tells you more about the effectiveness of your program than your first or than any lipid panel.  Your 47 HDL says to me you are not taking enough fish oil -- get it to at least 3000 mg of DHA and EPA combined per day (not just 3000 mg of fish oil).

    I screwed that up. Originally, my LDL was 155, now it's 95. My HDL is 49 (not 47, as I wrote).

    I have upped my fish oil 50%. I cut sugars out years ago, but I haven't tried cutting wheat out of my diet yet.

    What I'd like to do is try various diet changes and get tested after each change to see what's working, but I'm in New York, where the law won't let me go out and get my own cholesterol test.

    An Appeal for Support and Conformation of Adverse Effects

    My daughter has lived with ALS like symptoms for almost 3 years. The worst of the symptoms began when her simvastatin was increased to 80mg in 2008.
    Her MRI’s show LESIONS in the brain stem, specifically in the PONS area of her brain.
    Of course, her 4 physicians refuse to believe that statin is involved. They are all satisfied with the diagnosis of “Ataxia”.

    My Appeal is to all those who have similar brain lesions as shown and documented in MRIs. Please reply.

    To Anonymous (who left the comment above entitled Appeal For Support): You could go to www.spacedoc.net for information about ALS as a statin side effect. Print out the relevant pages and show them to your daughter's physicians. If they're not willing to consider that your daughter's debility may be linked to statins, why don't you fire them and find her a new doctor?

    Dr. Davis,
    My husband, 39, has a family h/o cardiac disease which took his grandfathers life at age 45.    Now, he has 30-50% blockage in a LAD artery at the mid point, and 30% blockage at the outer portion.  With diet and exercise changes, along with chinese herbs for the past 3 months his LDL has decreased from 132 to 119.  He has not added fish oil, niacin, Vit D, or red yeast rice to his regimin yet.  He has lost 15 # so far. Dr pushing Zocor.  What do you suggest?

    The statin-free life is real, it's just the matter of what you need more - life without statins or the regular tasty but unhealthy food that you love.  And I'm pleased to read that so many people care about this.  I was prescribed simvastatin and I've been taking it for some months as my doctors instructed although I felt fine. And only then (I know it was silly) I bothered to google for simvastatin side effects.  I was really scared  about myopathy because I often had muscle cramps even before I started to take simvatsatin and I never cared about it.  So I decided to stop with statins, even though my doctor insisted on taking them, and just excluded a number of products from my diet - meat, fat cheese, all cholocale, candy and white sugar, bread, cakes, etc, and totally switched to fruit, veggies, cereals, nuts, cottage cheese, honey, steamed potato, etc. I also used flax seed - the taste reminds me of fish, I love it. Now my cholesterol is normal, I lost many pounds and squeezed into my 10-year-old jeans. I'm not taking statins and I'm not going to get off this diet - i got used to it.  I also wrote about this (a bit outdated now) and this was not an attempt to persuade anyone in anything, just my experience. I understand that food is a real sourse of pleasure for many people and the diet is a real torture incomparable with statins.  It's just that I'not that kind of person and I hate being dependant on meds. Statins are just the price we pay for our pleasures.

    I really appreciate finding this source of information.  As a health care provider, I have seen a number of patients who have suffered needlessly because they chose to follow the statin lifestyle suggested by their PCP rather than changing their diet and supplementation.  I am convinced more than a couple actually died from the complications of the drugs.  Thanks for providing this resource.  I would like to offer your readers this article for their consideration.

    I had a patient who developed ALS after taking the same drugs.  Funny her ALS went away every time she stopped the drugs (3 times) but returned whenever she started them again.  Her doctor insisted that she would die of heart disease if she didn't take the meds.  Well, she took them and didn't die of heart disease.  I suppose he was right.  He told her to find another doctor if she wouldn't take his recommendations.  She paid with her life.  Supposedly she died of ALS.

    By the way, we were treating her for some of the other side effects, muscle weakness, joint pain, loss of balance.  During her medicated periods, she couldn't walk in her yard.  Her husband bought her a John Deere lawn tractor so she could visit her gardens and get about the yard.  When off the meds, she could ambulate on her own.  Tragic

In search of wheat: Another einkorn experience

Lisa is a trained dietitian. Unlike many of her colleagues, she has "seen the light" and realized that the conventional advice that most dietitians are forced to dispense through hospitals, clinics, and other facilities is just plain wrong

I know Lisa personally and we've had some great conversations on diet and nutritional supplements. I told Lisa about my einkorn experience and how I witnessed a dramatic difference between bread made from einkorn wheat and that made from conventional whole wheat. So she decided to give it a try herself. 

Here's Lisa's experience:


This past Friday, June 18th, I conducted my "Einkorn Wheat Experiment".

7 am

I embarked upon the journey of choking down, I mean enjoying, the hefty piece of Einkorn bread. Wow, was that bread dense!  It was a lot of work chewing.

(45 minutes after consumption, wanted to see what BG did a bit before the 1 hr mark)  BG 153

I was surprised with the BG of 153. However, it was good to see my insulin response is reactive and decreased BG 33 points in 15 minutes to end up with a BG of 120 1 hr after the bread.  

So, it appears my response is similar. A slight elevation of BG at the 1 hour mark, but not to the degree of conventional whole grain wheat bread.  

Of note, also, was the fact that I cannot remember the last time I ate a piece of wheat bread of this magnitude that did not make me bloated... not at all: No cramps, no brain fog, no headache and, did I mention not bloated?  

I believe you are on to something with tolerance of Einkorn wheat for those of us with wheat sensitivities, in addition to its apparent lower glycemic response.

Along with Lisa, I asked four other people with various acute intolerances (all gastrointestinal) to conventional wheat, i.e., people who experience undesirable effects from wheat within minutes to several hours, to eat the einkorn bread. None experienced their usual reactions.

Obviously, this does not constitute a clinical trial. Nonetheless, I find this a compelling observation: People like myself who generally experience distinct undesirable reactions to wheat did not experience these reactions with einkorn.

Note, however, that einkorn behaves like a carbohydrate. No different, say, from brown rice or quinoa. However, unlike modern whole wheat flour from Triticum aestivum,  in this little experience there were no immune reactions, no neurologic phenomena, no gastrointestinal distress--just the blood sugar consequences.

While this may not be true for all people consuming einkorn, it suggests that primordial einkorn wheat is quite different from modern conventional wheat for most people.

    Interesting. In your opinion, do you think Americans have genetically developed a sensitivity to wheat because of over-consumption? Or, do you think the current variety is just an inferior type?

    It would be interesting to try the same experiments with other early varieties which can actually be purchased retail, like spelt and kamut.

    wonder what the BG would have been had she done the high impact aerobic workout AFTER injesting the eikhorn bread.

    Carb injestion is suggested for high intensity workouts.

    Also wonder what the impact would be for a person with diabetes T1 or T2.

    Another interesting experiment could be to sourdough the eikhorn.  In the old days you couldn't just go to the supermarket and buy yeast. Sourdough is said to be healthier than normal bread because of the fermentation process.

Margaret is 73. She's a very good 73. She loves children and works full-time in a daycare. She manages her own household, goes to dinner at least once each week with one or more of her adult children. She is slender and has never been in the hospital--until she developed an abnormal heart rhythm called atrial fibrillation.

Most people who develop atrial fibrillation do so with no immediate identifiable cause. However, Margaret has been a widow since her husband died 15 years ago of a heart attack. She was therefore especially frightened of any heart issues in her own health. Her doctor also raised the question of whether atrial fibrillation might represent the first hint of future heart attack.

So we advised a CT heart scan. Score: zero, or no detectable plaque whatsoever. This put Margaret's risk for heart attack as close to zero as humanly possible. (Nobody is truly at zero risk for heart attack for a number of reasons. One reason is that people do irrational things like take cocaine or amphetamines, or they take too much decongestant medication, all of which can trigger heart attack.)

The heart scan settled it. Margaret has the sort of atrial fibrillation which likely simply develops as a result of "wear and tear" on the heart's electrical impulse conducting system and it has nothing to do with coronary heart disease or heart attack.

As that MasterCard commercial goes: Cost of a heart scan: About $200. Peace of mind: priceless.

    Not entirely true. With all due respect, Dr. Berman, who you have quoted elsewhere in you blog, has stated the following....

      â€œIn symptomatic patients, a calcium score of even zero does not sufficiently rule out the possibility of having an obstructed coronary artery, which was the case here.” Berman suggests that the coronary CTA may become the test of choice in symptomatic patients when the diagnosis is unclear."
    By the way, whatever was the upshot regarding the 'low dose CT angioplasty'? Safe? Revelatory or not?..... Dave in Chicago

Periodically, I'll come across a knuckleheaded report like this one from Minneapolis:

Marathon Man’s Heart Damaged by Running?


Of course, the obligatory story about how a cardiologist came to the rescue and "saved his life" with a stent follows. In other words, a stent purportedly saved the life of this vigorous man with no symptoms and high capacity for exercise.

Does vigorous exercise, whether it's marathon running, long-distance biking, or triathlons, cause coronary disease? Should all vigorous athletes run to their doctor to see if they, too, need their lives to be "saved."

Let me tell you what's really going on here. People with the genetic pattern lipoprotein(a), or Lp(a), tend to be slender, intelligent and athletic. For genetic reasons, these people gravitate towards endurance sports like long-distance running. Lp(a) is a high-risk factor for coronary disease. It is the abnormality present in the majority of slender, healthy people who are shocked when they receive a high heart scan score or have a heart attack or receive a stent. (I call Lp(a) "the most aggressive known coronary risk factor that nobody's heard about.")

The association between endurance exercise and heart disease is just that: an association. It does not mean that exercise is causal. Having seen coronary plaque detected with heart scans in many runners, virtually all of whom demonstrated increased Lp(a), I believe that Lp(a) is causal.

Unfortunately, the man in the Minneapolis story, now that his life is "saved," will likely be advised to take a statin drug and follow a low-fat diet . . . you know, the diet that increases Lp(a).

    I get the point about the Lp(a).  

    But I wonder about the hormonal effect of a *lot* of running.  Does all that running that raise cortisol levels?

    Does the carb-loading that many athletes practice (increased insulin production) contribute to increased disease risk?

    I totally disagree. While, exercise at moderate amounts is obviously very beneficial, extreme amounts of exercise are counterproductive. Too much exercise causes a person's body to age much quicker.

    Why? It is a fact that extreme amounts of exercise causes one's body to utilize massive amounts of oxygen. Too much oxygen has been proven to cause massive free radical production. This is seen in hyperbaric chambers.

    The same thing happens when a person eats too much. The food we eat turns into energy ("oxidizes") and in turn produces massive amounts of oxygen. That's why caloric limitation is the only thing that has been absolutely proven to extend life and slow aging.

    High free radical production equals aging (damage to cells). While eating in moderate amounts is healthy and necessary, too much is bad. The same thing applies to exercise.

    So what you're basically saying is that everyone should have a Lp(a) test, even if we are of healthy weight, athletic, and  even if we follow the TYP program?  My GP already thinks I'm a hypochondriac, lol.

    "Exercise at moderate amounts..." now that phrase will conjure up as many definitions as there are people who exercise.   No one would think that a daily 30 minute walk would yield nearly the same level of health benefits as a daily 30 minute intense weight training session.  But then it's amazing how many mainstream "fitness gurus" designate the former activity as all that anyone needs.

    Okay, after we have a couple blood tests, and if they show elevated Lp(a), then what?  The only accepted treatment is 1 to 2 grams/daily of nicotinic acid, and maybe antioxidant supplements like C, E, and N-acetyl-cysteine, which I already take.

    David, any references to back this theory up? We all know there must be a balance between enough (eliciting a hormetic response) and too much (damaging the body) exercise, but I'm not sure there's any evidence actually showing where the *exact* border lies, i.e. proving that X amount of exericse accelerates intrinsic and/or extrinisic aging while Y helps.

    "Extreme exercise" - whatever that is - causes accelerated aging?  And the evidence for this is...?

    And I am also to believe that "over-exercising" is akin to over-eating?

    I have heard such nonsense before. It just surprises me to read it here.

    I have personally put over $100,000 of my personal money to put this website up. Programmers need to be paid, though none of my staff are paid. There are also the costs of doing business, e.g, attorney fees, etc. This is a fairly costly operation, though nobody gets a salary.

    So I am personally shocked by attitudes that "I'll only do it if it's free." Free is what you get at WebMD, where the information is paid for by the drug industry. We do not take any money from the drug industry nor supplement industry. We pride ourselves on providing unbiased information. But somebody's got to pay the bills. I personaly got sick and tired of it. Putting in many hours a week for no financial return is my contribution.

    Is there evidence  that immediate post-exercise "damage" is predictive of long-term adverse outcomes?

    There are actually many other studies that have well proved that the more exercise one does, the more free radical pathology (cell damage/oxidation) occurs. Just how much is enough, no one knows exactly, but we do know it dramatically increases once our antioxidant enzymes (eg. glutathione, superoxide dismutase, etc.)are overtaxed.

    Now, how does does oxidative stress and cell damage correlate with degenerative diseases? Well, two reasons, which are somewhat based on logical assumptions:

    1. Cell damage to the artery wall is the essence of atherosclerosis. (Many actually suggest that oxidation is a chief player in inflammation).

    2. It is generally agreed by all that caloric restriction dramatically increases lifespan. The 'Caloric restriction,' principle is not necessarily related to being overweight or obese. Neither is it related to healthy or non-healthy food consumption. Neither is it an overeating/under-eating issue. Caloric restriction is simply limiting the amount of energy producing foods (especially carbohydrates) for anybody, thin or heavy, which in turn, drastically impacts the health of that person.

    The question we have to ask ourselves is this: Why? Why does caloric restriction play a part in prevention of degenerative disease? One of the main answers scientists have suggested is oxidative stress.

    David, what you do is commonly called "mechanistic speculation" -- I'm not saying you are wrong, but strictly speaking your guess is as good as mine: the point still stands, there is certainly a line whereafter exercise becomes damaging, but none of the provided references helps to us draw that line when it comes to *long term* health.
    Damage per se is not a good marker of anything due to hormesis. Aging would be defined as a build-up of *long-lived* damage that cannot be fixed by the cellular machinery; it's a tricky definition of a tricky problem after all.

    Damn! This exactly describes certain members of my family. I suspect one of the danger factors in excessive cardio level exercise is that they then fuel it with massive carb intakes, thus undoing all the benefits.
