Following high red blood cells in urine my Dr. ordered CT of pelvis and abdomen. He is now sending me to a urologist.

My question is - should I start calcium oxalate kidney stones protocol?
(How do I know what type of kidney stone I have? - I am guessing urologist would tell me)

HISTORY: RED BLOOD CELLS IN URINE; hematuria
COMPARISON: None ;

TECHNIQUE: CT of the abdomen and pelvis is performed before and following
intravenous 100 mL of
Isovue-300. No oral given. ; This study was performed using dose reduction
techniques to include
automated exposure control and/or adjustment of the mA and/or kV according to
patient size.

FINDINGS: Precontrast imaging reveals a 7.5 mm round calculus in the right renal
pelvis. There is
surrounding thickening and hyperenhancement of the urothelium in the renal
pelvis and proximal right
ureter. There is no distal ureteral calculus or hydronephrosis on either side.
No calculi seen in the
left kidney.

After contrast, no suspicious enhancing lesion identified in the kidneys. No
striated nephrogram
observed.

Abdomen CT: Remaining major solid organs are intact. No adenopathy.
Gallbladder appears normal. No
mesenteric or retroperitoneal adenopathy. No dilated bowel.

Pelvic CT: Normal appendix. Normal-appearing bladder and prostate. Scattered
phleboliths are noted. No
free fluid or adenopathy. No enhancing bladder lesion or bladder calculus.

Lung bases: Clear

Bones: Thoracic lumbar spondylosis. Bilateral hip osteoarthritis.

IMPRESSION:
1. 7.5 mm calculus in the right renal pelvis without hydronephrosis
2. Associated surrounding pyelitis and ureteritis is noted; consider urinary
tract infection and
correlate with urinalysis.
3. Thoracolumbar spondylosis and bilateral hip arthropathy

njurkovi:

I believe the "stent" your doctor is referring to is a device he inserts (you know where, or maybe you don’t) to grab the kidney stone and remove it.

And "round calculus" I would guess means kidney stone.

Hearthawk, I understand the procedure; I just don’t understand the doc’s wording.

What does "is remarkable" mean - is this some kind of doc lingo meaning that it is his suggestion to do the procedure?

It basically means your exam is significant enough to call for the use of a stent (measuring 7.5 mm) to be used to remove your kidney stone.

there is no reasonable explanation on why the radiologist commented on a stent.
Stents do light up on CT.
Maybe he needs to re-read it.
Your PCP can ask request a re-read to clarify the stent comment.

Yeah, I concur with all the conjecture above.  I would just ask your doc to clear things up as I find the wording confusing as well.  I think you’d know if there was a stent up there, lol!

Is "high dose" something other than 3x400mg Mg citrate daily as recommended in the protocol?

That dose of Mg citrate would be reasonable.

Hydration is also key, just as adding water to a sugar solution will prevent the formation of rock candy crystals.
